TREATMENT OUTCOMES OF KAWASAKI DISEASE IN CHILDREN AT NGHE AN OBSTETRICS AND PEDIATRICS HOSPITAL

Abstract

Objective: To assess the outcomes of intravenous immunoglobulin (IVIG) therapy and the evolution of coronary artery lesions (CAL) in children with Kawasaki disease at Nghe An Obstetrics and Pediatrics Hospital (2024–2026).


Methods: A case-series study of 53 children with confirmed Kawasaki disease treated at the hospital from January 2024 to March 2026.


Results: All children received IVIG within the first 10 days of illness, at a mean febrile day of 7.3 ± 0.9, with no adverse effects. The first-dose response rate was 88.7% and 11.3% were IVIG-resistant; all responded to a second dose. IVIG resistance was significantly associated with the incomplete form (66.7% vs 21.3%; p = 0.036) and with more severe coronary artery lesions (moderate dilation 66.7% vs 4.3%; p = 0.001). After infusion, inflammatory markers (white blood cells, neutrophils, ESR, CRP) decreased significantly (p < 0.001). Acute-phase CAL prevalence was 56.6% (mild dilation 45.3%, moderate dilation 11.3%, no giant aneurysm), declining to 11.3% (predominantly mild) at weeks 6–8, with 80.0% (24/30) of initial lesions fully resolved.


Conclusion: Early IVIG within 10 days yielded favorable outcomes; the incomplete form and severe CAL showed a statistically significant association with IVIG resistance, although larger studies are needed to confirm these findings.
