Nghiệm pháp thử thách đường miệng (Oral Food Challenge – OFC) là tiêu chuẩn vàng trong chẩn đoán dị ứng thức ăn (DUTA). Nghiệm pháp này có vai trò quan trọng trong chẩn đoán dị ứng thức ăn qua trung gian IgE ở trẻ em, đặc biệt trong trường hợp test lẩy da và xét nghiệm định lượng IgE đặc hiệu không giúp chẩn đoán hoặc không thể thực hiện được. Nghiệm pháp này thực hiện trong môi trường bệnh viện, mang lại giá trị chẩn đoán và đánh giá sự dung nạp thức ăn.
Hai trường hợp lâm sàng dị ứng đạm sữa bò ở trẻ em được mô tả. Trường hợp thứ nhất dị ứng đạm sữa bò (DUSB) qua trung gian IgE được chẩn đoán dựa vào lâm sàng, test lẩy da. OFC được sử dụng để xác định tình trạng dung nạp sữa bò sau thời gian ăn kiêng. Trường hợp thứ hai DUSB được chẩn đoán dựa vào OFC thực hiện tại bệnh viện với sữa bò và trẻ phản ứng với nồng độ 3ml sữa bò.
