Đặt vấn đề: Hội chứng thực bào máu (HLH) là tình trạng đe dọa tính mạng do hoạt hóa quá mức hệ thống miễn dịch. HLH có thể là tiên phát hoặc thứ phát. HLH tiên phát thường phổi biến ở trẻ nhỏ do đột biến di truyền, trong khi đó HLH thứ phát có thể gặp ở bất kỳ lứa tuổi nào. Nó thường được khởi phát bởi nhiễm virus, bệnh ác tính, hoặc bệnh dạng thấp. Virus SARS-CoV-2 gây ra HLH thứ phát thường gặp trong giai đoạn nhiễm COVID-19 cấp, tuy nhiên, HLH thứ phát hậu COVID-19 hiếm gặp. Báo cáo trường hợp: Chúng tôi báo cáo một trường hợp trẻ nam 16 tuổi, sau 5 tuần từ khi mắc COVID-19, trẻ biểu hiện mệt, sốt cao liên tục kéo dài 10 ngày, các xét nghiệm đủ tiêu chuẩn chẩn đoán HLH và có điểm H score ở mức cao nghi ngờ HLH. Sau 7 ngày điều trị với kháng sinh phổ rộng mà không điều trị đặc hiệu HLH, trẻ hồi phục một phần và hết sốt. Bệnh nhân hồi phục hoàn toàn sau 1 tháng. Kết luậnluận: Việc xác định sớm HLH sau khi hồi phục COVID-19 sẽ cho phép xử trí kịp thời tình trạng bệnh.
